Insurance Plans

I am a member of the Blue Cross Blue Shield panel. If the particular policy you have covers acupuncture, I will be able bill your insurance company and you will only pay your co-pay. Please call the office and ask to verify your Blue Cross Blue Shield coverage. The receptionist will ask you for the following information:

Personal Information

• Your Name,
• Phone number,
• Date of Birth,
• Email address

Primary Insurance Holder Information

• Name
• Date of Birth
• Relationship to you

Insurance Information

• Group Number
• ID Number
• Insurance Plan Name
• Phone number (on back of card)

Insurance Discount Programs

I am also a member of 2 insurance discount plans which cover (American Specialty Health and Healthways). I offer a 35% discount to holders of AETNA, Cigna, Kaiser Permanent, and Health Net health insurance plans.

It is difficult to publish a menu of prices because of the different programs. Please put yourself first and don’t let cost be the obstacle that keeps you from taking care of yourself.
